Arizona Family Counseling accepts most AHCCCS health plans.  We do not accept commercial insurance plans for the following reasons:

• We want YOU to have choices in the counseling process.
• Insurance companies have panels with specific professionals on their list. We want you to be able to choose who you would like to see for counseling.
• You may also find that you are limited to a certain number of sessions by the insurance company you are affiliated with. We believe that this greatly hinders our clients’ ability to see the counseling process through.
• Many individuals have high deductible plans which require they pay out-of-pocket for counseling services.

• Health Insurance Claim Form

If requested, we can provide an insurance superbill for your counseling appointment at the time of service.  This superbill may be used by you to submit a claim to your insurance company for reimbursement.  You will still be asked to pay for your counseling session at Arizona Family Counseling, but you can request reimbursement from the insurance company for your out-of-pocket expense. 

If you would like to submit a superbill to your insurance company, we can offer a one-time tutorial regarding how this process works.  We cannot guarantee your insurance company’s acceptance of the reimbursement request, as they each have their own criteria for reimbursement.
